HEALTH DECLARATION FORM

To prevent the Local transmission of Corona Virus (COVID-19), by virtue of EO No. MR-05 s. 2020 entitled “AN EXECUTIVE ORDER DIRECTING THE IMPLEMENTATION OF PRECAUTIONARY MEASURES AT THE ENTRY POINTS OF THE PROVINCE OF ABRA”, all individuals, both residents of and visiting the Province of Abra are hereby directed to execute a Health Declaration Form to be submitted at each entry point.
